Perhaps, The IP is registered as "Bad IP" by sbl-xbl.spamhaus.org.
I've just disabled mod_access_rbl in this site.
Try to register once again.

Anyway, there are no documentations how to create plugins for sitemap.
Just refer source codes of a variety of plugins.
